Windows Login Popup in Custom-FBA

I implementedd an FBA website using ADAM authentication with SSL. Now
whenever I try to access site externally or internally, windows login
pop-up comes rather then FBA Login. Following is my Alternate Access

Internal URL - Zone - Publc URL for
http://portal - Default - http://portal
http://portal.domain.com - Intranet -
http://SqlFbaPortal.domain.com - Internet -
http://AdamMySiteFbaPortal.domain.com - Custom -
http://AdamUserFbaPortal.domain.com - Extranet -

All other site are working fine except red colored site keep asking
windows login rather then FBA login. This site access by those users
who is having MySite feature enabled. I have selected Authentication
Type as "Forms" in Custom Zone. Actually I want to create 2 extranet
site for ADAM Authentication, but It only allows 1 extranet site at a
time, that is why I selected as Custom Zone.

Please advise if what I am missing ?

If you are using FBA then your web.config file should represent the

<authentication mode="Forms" />

It should be "forms" on the extranet  vDir.

It is already in web.config. FYI I am using Custom Zone as I
mentioned in my previous post.

Is there a way to create  another extranet  alternate access  mapping?

I checked in web.config. It is set as <authentication mode="Forms" />.

Any other suggestion.

Are there any web parts or images on the page that come from other locations in
the environment? For example a picture from the _Layouts/images directory or a
CQWP aggregating content from a subsite? If these resources come from somewhere
in SharePoint that uses Windows Authentication then you will see the Windows
Login popup  when you try to login  to the FBA site.
