User with full permissions can not create pages in Pages document

I have a mysterious security problem. I am working with one user in
one of our departments, let's call the user Phil and the department
Finance. Phil and other members of the Finance Department Members
Group in SharePoint can not create a page in their sites Pages
document library. Additionally,

* If I change Phil to Finance Owners, he still can not create a page
(they have full control of their site)
* If I escalate Phil's permissions to site collection owners, he
still can not create a page
* I created another site and Phil can not create a page there
* I restored the entire site collection to another server farm,
again, Phill can not create a page

Phil can create lists in all of these places, but he can not create
pages, nor can he edit pages.

How can I troubleshoot this problem? Does this have something to do
with Phil's account SID? The security event log does not contain any
errors relating to Phil. I haven't seen anything in any of the
SharePoint generated logs.

I'd suggest checking the membership of the Approvers group. This sounds very
similar to the problem I had with the News site.

Check the permissions of the Master page and Layouts library

He likely does not have permission to an element required to create the
page.

I'll keeping looking at what is different about his account from the
other people.
