Is there a way to edit a custom running workflow created with workflow
designer?

Are you talking about updating instances of the workflow  that are already in
process? I do not beleive that changing the process in designer will
change existing instances. If there is a tool to do this, I would also be
interested.

If the WF is designed to kick off again from an ItemChanged event then yes. The
next time it re-runs it will grab the newest version of the WF you added
to the list.

Otherwise, no, because you would be going in against the WF when the thread is
in memory and/or parts of it are sleeping in the database. Directly editing the
database isn't supported and I'm not certain that you want to hack your
servers memory.
