I created a survey. All of my employees have contributor permission.
When they try to use the survey, they fill in all the info, then click
save and they are prompted for their password and then kicked out of
Sharepo int. As if they don't have permission to add to the survey.
(but they should since they are contributors). Permissions/security
within survey itself was not changed at all. Does anyone have an idea
why this might happen?

Be sure to grant them the ability to EDIT their own responses as well
in the general settings for your survey. For some reason, it does not
allow them to add  new responses if they cannot edit them.

In creating a survey, some of my questions have checkboxes for the
response. If a user selects a checkbox "none", I would like to hide
the rest of the checkboxes for that question so that they cannot fill
in any additional.

Is it possible to customize a survey. I don't see any options for this.

We're using SPS 2003.

What I'd like to do is to create a survey  where the results are
anonymous, but where I can see the names of users who have completed the
survey. Is this possible?

And even better, I'd also like to see a list of users who have *not*
completed the survey.

I'm not sure you stumped the group, but my first thought was how can something be anonymous but still capture who completed it - isn't that the opposite of anonymous?

Well, it would really be semi-anonymous, in that sense.

The goal is for me to have a survey  that I can confirm has been completed by all employees in the district. So in that sense, it wouldn’t be anonymous; I need to know who has and hasn’t completed the survey.

The goal is *not* for me to be able to match survey responses to specific, identified users. I would know that Jane Doe had completed the survey, but I wouldn’t know what answers she put. So in that sense, it would be anonymous.

This may be beyond what SPS can handle; I may have to look for a more robust survey system.

Well, if you set the option to "No" for "Show user names in survey  results? " I don't think you'll be able to retrieve the users that have hit the site without making it cludgy ... you would need to itterate through all the responses programatically and retrieve all the user names for the survey results.

Don't think you are looking for something like that ...

From a development perspective, it’s a custom web part. You can just make it look like the regular survey, but not display the user name. Also within that custom web part you could just add  a new option that only admins have access to and in that function you display who hasn’t taken the survey  (based on who has and the membership of the site).

You *might* be able to do some fancy DVWP and then remove the name, but DVWPs are read-only so you won’t get onto to do data entry for you.

In 2007 you would be able to do this OOTB.

I recently created  a survey  on my Team Site and I would like to move
it to my department's Team Site.

How do I do this? I don't see any move or import options.
