SPS2003 - Survey Question

I am wondering if there is an easy way to create an "Acknowledgement"
button on a WSS 2.0 site whereas all the user does is click the button
and it logs an event in a sharepoint list?

Basically, we have a page that users must go to, read a policy and then
click the button. That's all we want it to do.

Originally I created a single-question survey on a wss site, put a html
button on the site and just linked the button to the "take this survey"
button on the survey. The user clicks the button, is taken to the
survey, clicks the box and then has to click Save and Close on the
survey interface. This is sloppy and choppy and the users will not like

Is there an easier way to do this in WSS, so the user can just click the
button and it logs that they clicked it and their username, email etc.
is stored in a list?

Without major customization, is there a way this can be done easily?

There isn't any way I can think of to do this without custom code, but a
custom .aspx page  that uses the OM to write the list entry and redirects
back to a specific URL would be pretty to create.

Do you know of any way to get more information in the survey  results data?

The only information i need from the results data is the date/time modified, the
username and their email  address. Unfortunately, the only thing that shows up
in the survey results when exported is the display name and their survey answer.

Is there a way to also have it export their email address and the timestamp?

The email  isn't going to be available without extra work (or prompting
the user  for it) as it's not stored  in the list itself and would need to
be pulled from the user profile via code. The timestamp is stored in
the list, but it sounds like you might need to modify the export view of
the survey  list to include the column (it sounds like it's not there by
