SPD Workflow Working Halfway

I have problem working with SPD work flow.

I want to create 3 step approval for our company requisition form.

It's similar to this article:
cycogeek.fiesta25.com/.../Create-Custom-Approval-Workflow\

My problem is the work flow work until first step an the it say completed..

Did anyone having similar problem like this.
