SP Designer Workflow - How to add attachment to the custom form crea

by "Collect Data from user" form.

Is there a way to do this?

If not, can I switch the form to use and copy the logic to mark the source item
as complete. (like in the "Complete Task" button.

If you build your workflow  off of a Document Library then the "Related list
item" link on those custom  task edit forms is the link to the document.
