Sharepoint Document Library Permission"File Not Found " Error!

I'm anting having problems granting permission to sharepoint 2007 doc library.
Anytime i add a user / Group, i get an error message which says file not found.
i am able to remove users but not add users.

Also, if i create a new document library, it does not inherit the parent sites
permissions.
