How to remove built-in fields in sharepoint custom forms and add my

I've downloaded a sharepoint application template, "Bug Database" and
installed it. It comes with a "Log a Bug" custom form but there are
some built-in fields on the form such as: priority, severity, related
bugs, etc., which I don't need and I want to add some new fields of my
own.

How do I remove those built-in fields and add my own?

The fields  on the form  must exist in a list in order to be displayed. So the
first step to adding new fields is to add  them to the list that is behind the
form. Once that is done you should look at just dropping the fields you don't
need from the default view and adding the new fields to the default view of the
list. If that doesn't work you may need to edit the page in SharePoint
designer.
