Receive error 0x80070002 when creating Document Library or "Team Sit

I've run into a problem with one of our WSS 3.0 installs where somewhere
between four weeks ago and today an issue has crept up where we are no
longer able to create new sub-sites or site collections without the page
returning with a "File Not Found" error.

Through additional testing, I realized I couldn't create the sub-site if
it was a "Team Site" template, but it would work with "Blank", "Blog",
etc. So I was working with a user to provide a temporary work-around of
creating a "Blank" sub-site, and adding the lists/libraries as needed.
When we tried to add a Document Library, we received the "File Not
Found" error. However no issues with creating a Form Library, Calendar,
etc.

I opened up SharePoint Designer to see what kind of feedback it would
give me, and when I try to create a "Team Site" sub-site or a Document
Library I receive the following error:
The operation failed because an unexpected error occurred. (Result Code:
0x80070002)
I have been comparing hives (using WinMerge) with another WSS 3.0
install, and don't see any major differences. The site definitions are
the same. I also had our DBA to a database corruption check on all the
db's, and they came back fine. I have been troubleshooting this most of
the day yesterday and this morning, and there's just not much out there
that's pointing me in a good direction so far. Anyone have ideas?

Here is more information on this specific server:

* WSS 3.0 SP1 - although the issue occured before SP1 (did a rollback
on the VM to pre-SP1 to test, which was installed yesterday)
* Single-server install (integrated database)
* Installed with the Microsoft Application pack
* Installed with Adobe iFilter 6.0
* Custom theme installed

I would really appreciate any help/advice/pointers in troubleshooting
this.

In trying to think through what could be the problem, I'm coming up
blank. Obviously, something is wrong. Do you have a change/control
management log that you can reference on what has changed on the server
during the last 4 weeks?

If you copy in the team  site definition from another installation, does
the template  work?
