Radio Button Infopath Web form - Survey

I have created a group of radio button (Five Buttons ) for a survey.on an
infopath web form. They do not want to use the standard survey features.The user
has to choose a rating between One to Five. Now the field name repeats for every
radio button in that group.

How do i hide that name and give each of the radio button a rating of one to
five so that if one button is clicked the other button in that group is not
clicked.

Not sure if I am understanding the issue correctly or not, but here's how I had
radio buttons  to a form  (Using IP2007). First, drag the option button  control to
the form. From the popup window, select to add 5 buttons. They all get added in
with the same label (Field 1), bound to the same data source (myFields:field1).
Then, change the text for each radio  button from "Field 1" to the values you
want (1,2,3,4,5 - or Good, Somewhat Good ,Average, Somewhat Poor, Poor - you get
the idea). By default each radio button gets assigned a value 1-5, so when you
propagate that control to sharepoint, it gets the value selected.
