In-Place Upgrade Difficulties - 'Cannot insert duplicate key in object

The other day I posted a message indicating that I was having difficulty
with some errors during an in-place upgrade attempt from SPS 2003 to
MOSS 2007.

The upgrade.log shows a few errors, and then the "new" MOSS 2007
installation seems to get stuck. I've noticed that the "Default Web
Site" Share Services Provided is listed as "Provisioning in Progress"
Furthermore, there is an option for "Finalize Upgrade" on the Operations
page of the Central Admin Site.

Sooo... the question is: What would cause this problem?

The first of the errors in the upgrade.log is:

[InitialFarmCreation] [] [ERROR] [3/15/2007 4:44:39 AM]: No local
IIS web sites are serving the v2 SPVirtualServer named Pre Pub. This
SPVirtualServer cannot be upgraded.

...but I assume that's not a big deal as that's not a production item,
so I'll just delete that next time.

The error that I suspect is starting the failure is:

[UserProfileV3M1FC1Upgrade] [] [ERROR] [3/15/2007 5:00:14 AM]:
Violation of UNIQUE KEY constraint
'IX_DataServicePropMap_ServerIDPropName'. Cannot insert duplicate key in
object 'dbo.DataServicePropMap'.

The statement has been terminated.

In fact, I see a few of these... but I have no idea what they mean...
and, therefore, what the cause is.

Can someone point me in the right direction?

I am not sure what would be causing the errors, but i might be able to help
you get past this problem. I had a similar problem with the SSP being stuck
provisioning during an upgrade. Have copied and pasted my final resolution
on this below.
