Periodic Master Page File Not Found Error

I have a custom master page - nothing too sophisticated, just some HTML layout
changes - deployed as a feature on the SharePoint farm.

The sites where the master page is used are accessed globally. Periodically,
certain users in certain locations will receive a File Not Found error message
in SharePoint when going to the home page of a site that uses the master page.

The SharePoint log files in the 12 hive receive the following error message at
the same time that the File Not Found occurs:
Cannot get ghost document:
Features\MasterPageFolderName\MasterPages\MasterPageFileName.master
Unknown SPRequest error occurred. For more information:
0x80070002

We have not yet nailed down a pattern for when this occurs. Certain users will
be able to click around on the site for a while, but eventually encounter the
error message. Other users seem to never encounter it.

Has anyone seen this before, and do you have a suggestion on how to overcome it?

All web servers have the MasterPage deployed?

Why don't you try to use SharePoint Designer to add a new customized
masterpage to the masterpage gallery instead of deploying it as a
feature?

Is this a multiple WFE configuration? If so, is it possible that this
is only happening when the master  page is being accessed  from a specific
WFE?
