On the web  server (Where sharepoint is installed)

For WSS, it is the SQL Server. For Portal, it would be the server  performing the crawl.

So for most SharePoint Portal installs you need to install it on both.

Index server, not the web  server.

Nope, just on the Index server.

If your users only use the Portal search, installing the iFilter on the Indexing server  should be fine.

What about if the user is working in their WSS site (that the Portal is indexing) and they do a search from there? I believe that unless you have the iFilter installed  on the database server, that your search results will not return documents that require the iFilter to find.. This would give users inconsistent search results, due to WSS using SQL's full-text indexing vs Portal's own Search gatherer.
