Page Modified Must Refresh Error

We're seeing quite a bit of the:"This Page has been modified since you
opened it. You must open the page again."
errors as of late and I can't really explain it. I know it is a legitimate
error but I am even seeing it if I just ask to see the Page Editing Toolbar
on a page. More oddly, I have seen it on occasion when saving a normal list
item.

Has anyone experienced this or have any insight?

I thought it may be because of the corresponding page  is detached from
the site definition.
