You are asking several different questions. First, what you see in the default view is the Name, not the title  field. The Name field  displays the file name of an item and thus can't be removed  from the list. You can remove it from the view though and add the Title field. Keep in mind that only the Name field displays a context menu. As for changing the name and the title, you can do both when uploading files one at a time. The Properties dialog doesn't appear when you upload  multiple files at a time.
