"Modified By" column bug?

We have experienced a very weird situation in wssv2. "Modified By"
column for several document in the document library always show
modification by 1 specific user. But the user didn't modify the
document or open the document at all. We turn on the version history to
check, and there are no changes to the document.

Is there a bug somewhere?

May be u didnt enable, create versions option for the document library.

I have enabled the version history for the document
library.

I found that ONLY NEWLY uploaded document will be
modified by this specific user, userA, who is the
adminstrator.
but userA didn't change the document, why there's a
new version generated in the version history?

what is using userA account when new document is
uploaded?
