List forms - automation of domain user creation form

I am trying to brainstorm on a way to automate the process of
employees requesting new user creation. Right now we have a form that
the departments fill out by hand. They then inner-office it to IT and
then when the user is created and hardware/software is allocated it
them moves on to facilities and then HR.

I would like to figure out a way to create a list that the initial
person will fill out. It would then fire off an alert to the IT staff
to let them know there is a new user. They would then fill out their
part and it would fire off an alert to either HR or Facilities. HR
would then Review anything they would need.

We don't have InfoPath so we can't use that. I know a list tracks who
modified lists and when so that would be a good tracking tool.

If you guys have any other suggestions or places to look let me know.

You can play with an Issues list. Issues allow you to change who the
item is assigned to, which will then fire  off an email to that user.

This seems as if it will work. One question I have is how does the
built in comment field work? I like how it "blanks out" every time
you edit the item. In my form  I have other areas I would like field
like this. Other multiple line fields keep the same information in
them when you edit the task.

For instance I have drop downs to change status, ownership etc. I
would like certain areas to allow users to comment on why they changed
the status of the form or why they reassigned it to someone else. It
is nice to have the quick view of the form changes and list  the
comments ever time it was edited.
