Link survey response to a task list item

We have a Service Requests system set up and it has come to our attention that
we need a way to follow up with our department's customers the results of the
Service Request. One thing I thought of immediately was a survey, but what I
wanted was to link a survey response to a specific service requests list item.
When a service request is completed, the requesting user gets notified of the
status of the request using a workflow created in SPD. What I would like is for
that e-mail to include a link for the user to fill out a "satisfaction" survey
in a way that we would be able to track a survey response back to the service
request. Has anyone done anything like this, and how would I go about it?

Yes it is possible to do this, you need to set  up a workflow  in SPD with a
"Collect Data From User" action. This will allow you to ask the questions you
need and assign this as a task  to the specified user.

Each of the questions will then be a field in the task. You can then add a
second workflow to the task list  to process these responses when they come in...
