We have an MOSS installation with forms server. The apppool is crashing
very frequently almost twice a day.
But there were no exceptions logged. So, I added the unhandled
exceptions http module and now, I see this error.

This error happens and then, Sharepoint app pool crashes. So, users are
unable to access the server.

Any ideas as to what could cause this error? any pointers as to how to
locate the issue?
