ISAPI authentication filter works sometimes

I wrote an ISAPI filter to work with a single sign-on (SSO) solution.
It works with our SharePoint site up to a point.

- Our "AuthFilter" obtains user identity from HTTP variables created
by SSO.
- AuthFilter processes SF_NOTIFY_AUTHENTICATION.
- AuthFilter then maps user to a Windows account in Active Directory (AD).
- AuthFilter fills in pszUser and pszPassword.
- Control passes back to IIS.

The above works as it should for non-Sharepoint pages that require
Basic authentication.

The above also works on our SharePoint site to access the home page
and some other resources when SP is ocnfigured for no anonymous access
and "allow access for all authenticated users"; furthermore,
authenticated users are members of Contributors.

However, these supposedly authenticated users cannot create a
Discussion, although they can read it; they are prompted for Basic
auth. And they cannot even view photos in a Photo Library - again they
are prompted for basic auth. Once they authenticate with Windows
account, OK.

I note that the HTTP server var, REMOTE_USER is empty and David Wang
says probably I need this to be filled in.
