Inconsistent Survey Behavior & Permissions

On a MOSS site we do a good bit of sub-site templatizing, and object (list,
DocLib) templatizing. We use these to stamp out new sub-sites. All from the UI.

1) saved survey as a template
a) added survey template to a different sub-site WF9, changed description to
say WF9, gave permission to Sandy & asked her to respond -> no problems
b) added survey template to WF9a (a sub-site of WF9), changed description to
say WF9a, gave permission to Bob & asked him to respond -> no problems
2) created a sub-site template with the survey included (from survey template)
and then created a new sub-site using this ss template
a) changed description to subsite's name, gave permission to Maggie and
asked her to respond -> log on problem and error submitting
b) add another subsite from the same ss template, changed description and
gave permission to Sharon and asked her to respond -> no errors

Any idea what's going on? We are very careful giving correct permissions when
doing this sort of testing.
