Home link issue with the migrated sites

I'm having issue with the migrated sites from 2003 to 2007.

%23SVRREL%23 is getting added before the default.aspx to the top navigation
bar from the Lists pages (AllItems.aspx, DispForm.aspx, NewForm.aspx etc).

When I hover over the Home link from the top navigation here what it looks
like.
