Getting an error message when trying to view an approval workflow

I am getting an error message that states,

System.Xml.XmlExcep tion: 'ows_TaskStatus' is a duplicate attribute name.

This is for a MOSS installation.

This is what happens when a user starts the "collect feedback" workflow for a
document.

The tasks gets assigned, the user that the task was assigned to gets an e-mail,
and milliseconds later gets the same task e-mail again causing a conflict in
Outlook. If the user chooses to keep one item and then complete the task as
usual, everything seems okay, until trying to view the comments on the server.
An error message comes up saying that "There was an error while loading the
form." And the user is given the option to try again, or close the message.
Trying again only brings up the same error message dialog. The user also gets
the option to "Show error details." What is posted below are the results of
that. Hopefully someone can figure out what happened.

Upon doing some further research, any out of the box workflow  is unviewable once
it is completed. Giving the same error  message.

This is a major issue. I have completely rebuilt the development server  where
this issue is happening. And the problem remains? No one has seen this?

This issue has now been reproduced in a brand new installation. I rebuilt the
entire server, including reloading the OS. All from scratch. Google is no help
as although I am not the only one with the problem, no one seems to have an
answer.
