fldpick.aspx, Error=No two choices should have the same ID

I modified a feature to install some new columns and now I get the error

Application error when access /_layouts/fldpick.aspx, Error=No two
choices should have the same ID at
pare(PickerItem c1, PickerItem c2) at
System.Collections.Generic.ArraySortHelper`1.QuickSort[TValue](T[] keys,
TValue[] values, Int32 left, Int32 right, IComparer`1 comparer)

Now remind me: Say I have a field element already within a feature, and
I want to add some new choices for example to the choice list. Do I have
to install a new field element with a new GUID, or can I make changes to
the feature elements file and just do an installfeature -force
activatefeature -force.

It appears to work in the site columns gallery, but then when I try to
add columns to a content type or list I get the error above.

Now I can't undo it and it seems my farm or site is corrupted, and I
always get this error.

I realize the changes don't propagate to the document libraries when you
install column features like this, but how do you install features over
top of old ones without running in to this type of error. This seems to
be a flaw with every feature I try to maintain and install.

I would like to deactivate, uninstall, install, and activate every time,
but sharepoint won't let you once a content type / column etc is in use.

Has anybody documented what I SHOULD be doing?

It may have been because I changed the static name so it wouldn't have a
space, but didn't change the GUID. Could this be the case?
