FAB40 Helpdesk Template

Is anyone out there working (or have worked with) the Helpdesk Template from the
Fab40 package? I am interested in what others have done with this. Besides the
OOTB FAB40 Helpdesk template, does anyone have a modified version of this
running at their location? How do the users like it? How
do the service requests get assigned to IT (automatically or manual)? What
changes were made to the Service Request screen that the user enters the ticket,
if any?

I have been working with this template and have gotten to a point that It is
needing some Customization, i think with workflows. I have this simplified
enough so the users enter minimal data, and hoping to get it so the users'
manager gets an e-mail to "approve or reject" the request if it is a certain
type of request. I know what I want to do (on paper), but transferring it to
SharePoint Designer has been challenging (and have had no luck).

I have modified  this template  for two companies and added workflows, reminder
alerts, escalation based on severity and some other cool things to make this
more automated.
