Extranet SharePoint Site

We currently have an SharePoint intranet Portal site and want to extend
project sites within the Portal site to external users. How would we
do that without having to have external users have AD accounts? We
have an ISA server in our environment. Would we be able to set have
external user accounts live in a SQL database? How would we manage
accounts? Is there a registration webpart out there?

We have done almost exactly what you are describing. We used a dual
authentication solution, where our external  users use Forms Based Authentication
that checks against accounts  created in a SQL database  and our internal users
authenticate against the AD. There are plenty of links that describe how to do
this step by step. Hope this gets you going in the right direction.

We set  up something similar to this. Basically, we had an
internal.domain.com web app using AD authentication that we extended to
external.domain.com using forms based authentication. It still seems a
bit clunky to me but it works. If anyone else has tips on this, I'd be
interested in hearing them too.

There's a great article here that got me started with setting up the
FBA authentication.

From there, I installed this feature to get some sort of user
management for the external  users into Sharepoint. Note: We have very
few external users  so the feature is only available to a few
administrators in IT.
