To test the extranet  from the Internet:

From the computer connected to the Internet through an ISP, type
For example, type http://sharepoint.extranet-server.com

The default Web page (default.htm) that you created in the previous
section should appear.

If the default Web page appears, type
For example, type http://sharepoint.extranet-server.com/Marketing.

The dashboard site for the Marketing workspace appears.

I don't think so. It's a completely separate issues.
First of all - FQDN have nothing to do with MS SPS or IIS or any
application server. You can have different DNS names, which can be
resolved from outside or inside your network, for example
www.extranet_server.com or extranet_server.com or
sharepoint.extranet_server.com - whatever, those names support and
resolution should be provided by your ISP or your local network DNS
server (in case you want to use your private one for intranet network)
So, when you have those DNS names you can configure your web application
server (IIS) to share web applications.
Shortly, you can use your SPS dashboard through the "default web server"
HTTP service on port 80 or as virtual web with some special settings (I
prefer to use it this way) - it's all up to you.
MS provide a couple of tools to simplify this deployment. First one
second - www.microsoft.com/.../RapPort.asp
(it's much more then a application configuration tool)
