Document Library with document title linked instead of document name

To the best of my knowledge the menu block in sharepoint can only be linked
to the Title field. Its hard coded that way and I don't think you can
change it. I've seen people try, but never heard of a succesful attempt.

Can this be easily implemented on a global scale?

If so, where do I start?
