Doc Icons Not Showing.

I ended up with only the white rectangle in place of an icon as you mentioned
and when it did happen it was always when working with Adobe files. It appeared
that the metadata wrapper got separated from the actual file content. As for a
fix I am still stumped as to why the link between the two broke.

If that's not it you could try checking to make sure the column is set up to
show with Title Linked to File.

I will be watching to see the outcome.

If you are talking about icons  showing inside document libraries then this is
normal. The icons associated with documents in document libraries are
controlled by an XML file  called docicon.xml which is stored in the Template\XML
directory of the 12 hive on the SharePoint server. There is an entry in that
file for each supported file extension specifying which icon  graphic should be
used in a document library for files  with that extension. adobe  files like PDFs
aren't included in the list by default so they default to a plain white  page
icon. You can modify the file to add a 16X16 graphic icon of your choice. Tead

file.

check to see if there is any mal-formed xml in the docicon.xml file.
I had the same issue and I fixed it by adding the missing "/>" tag.
