Dependent list WebPart?

We need this functionality desperately.

Pretend we have a document library which has a column which is
populated via this lookup list:

State City

Alaska Anchorage
Alaska Fairbanks
Alaska Juneau
New Jersey Long Branch
New Jersey Neptune
New Jersey Asbury Park
New Jersey Kearny
Maine Orono
Maine Bangor
Maine Fort Kent

And—pretend this lookup list is hundred of records long.

On the upload document form, we want 2 fields—one for state and one
for city and when the value is picked for state then the values for
city are FILTERED, yielding only a few values out of the original

Can someone please get us started in the right direction?

The best information I've found (but not had time to consume) about
modifying elements on pages like the upload  page is in Stacy Draper's
SharePoint 2003 Advanced Concepts book:

If memory serves, Stacy wrote about this in chapter 04.

We are doing this by adding JavaScript on Upload.aspx and EditForm.aspx page. When you select a State, it will change the values  in the second drop down list  dynamically.

This can certainly be achieved using JavaScript too. On a
given form, you can manipulate the select list  options as objects using
client-side JavaScript events. Keep an array of the initial options.
When the user selects the value from the first list, clear the second
and repopulate it with only the valid options related to the selected
