Custom workflows in windows sharepoint services 3.0

I have created a custom workflow using WF (workflow foundation) and associated
it to document library in one of our sites. I have set the workflow to kick in
automatically. This works fine. However, if I associate my workflow to one of
the available content types in a document library and set it to run
automatically, it works the first time, but does not later on. The status of the
workflow column shows 'Failed to start(retrying)' and here is the error from log
files

It's hard to tell without looking at your code. But given the error  I would
start looking for some field that you are expecting to use from the Initiation
form that is not being instantiated on the second run. If you associate  it with
the content  type can you start it manually multiple times?

Yes, I can start it manually multiple times without any problem.
