Creating a Feedback Form

Hi. I am very new to Sharepoint and trying to create a basic Feedback Form within Sharepoint. I do not have access to Sharepoint Designer or Infopath in my company and need a tutorial or step by step instruction on how I can create a Feedback Form. This is my vision for the Feeback form:

Question 1: What type of Feedaback would you like to give:
Drop down Option: Design / Information / Overall site

If they chose Design questions related to design:
Do you think the color scheme needs to be changed? If so, what color?
What should be added or taken out?
Genreral Comments;

If Information:
What information would you like added on site:
which part of the site would you like it added to:

If overall site:
General text box for comments.

Each of these options should only pop up based on what they chose on the first question.

Create a custom list and use it as a online form

Of course there are a few details you need to set up with SP Designer, you can download it for free at Microsoft.com

When you create a list just go the the settings of the list and add fields, those fields can you drop down, multiple choice, etc

Then go back to the list and click on new
copy the URL of the list and link it on your web site so people can go and fill it out.
