How to createTop-Level Site from Custom Site Template

I want to use a custom site template when creating a new top-level
site. I read that a site template can be placed in the central
template gallery using a command line tool on the web server ...

- where is the central template gallery; how would I navigate to it
- what is the command line tool or statement for accomplishing

Stsadm -o addtemplate...

It has no UI so I call it the Custom Site template  gallery in the sky.

A couple of notes. You'll need to do an iisreset after the stsadm for the
template to show up. Once you do it will show up for top level sites and webs,
and there is no way to hide it like you can stock templates in Webtemp.xml.
