After we created 50 lists in 50 different sites...

They decided they wanted different values in a choice list.

Do I have to visit 50 different sites and modify the choice column to
add the new choice options?

I see in the database where the choice field is assigned to a column in
the userdata table. But I can't figure out where the choices are
stored.

If you created  the column  as a site column in the top level site of the
site collection and used it, then the answer would be no. If you
created it by adding a column to each site then the answer is Yes. If
you do it by modifying the database  directly you will void your
warranty. If something goes wrong MS will not support you.
