Can't save content...missing a placeholder in my template?

I've created a masterpage and pagetemplate. The page template has a HTML
field on it.

When I go to create a new page on the site with this masterpage and
template, I can see the HTML editor, but I don't have any of the
publishing tools (ie, SAVE, APPROVE, etc.) show up on screen.

If I use the default masterpage, however, I do get the publishing
toolbar.

I'm assuming I'm missing a particular placeholder in my template. I have
no idea which one, though. Anyone know what the 'publishing page
toolbar' placeholder is called?

Also, is there a guide to all of these place holders somewhere?
