Can't Connect to Database

All of a sudden this afternoon we are experiencing this error:

Unable to connect to database. Check database connection information and
make sure the database server is running.

The servers are on the same subnet and I checked and the SQL server is
running. I was adding Items to a list on the portal site and all of a sudden
the error showed up.

I've recycled IIS and checked connectivity over the network.

We've already checked oledb32.dll according to KB 883547 and it is version

Assuming that you've confirmed there's no DNS, etc issues with the database  name
as configured in the SPS admin pages? Any chance that a password changed for
one of the SQL or SPS service accounts?

You should make sure that the Application Pool Identity didn't change.

Also check  if SP1 was installed and make sure that if yes that both WSS
and SPS SP1 was installed

Yes SP 1 was installed for both wss and SharePoint after completely
rebuilding the server  because SP 1 broke it.

We had just started getting it configured when we had to change the names of
the SQL server and SharePoint server to comply with naming conventions for
our impending migration to Active Directory. We managed to get through that
and were starting to populate document libraries and lists.

I'll check  the App Pool.
