Approval workflow and versioning in a list library

I am using the contacts template from the 40 application templates for MOSS
to share contacts within the organisation.

I have enabled the approval worflow which requests approval from a
designated user whenever an item is added/modified. Also enabled is the
versioning setting and requires content approval prior to being listed for
all to see.

My problem is everytime the designated user approves (approve/reject) the
item it shoots an email (and creates a task for the designate); the item
however doesn't get approved.

Maybe a dumb question, but does the user  have sufficient privileges to
approve items in this list?

Also, are there any errors in the Windows event viewer or SharePoint log?

If you meant the Post-completion workflow  Activities (it is checked as is
evident below).

After the workflow is completed:
Update the approval  status (use this workflow to control content  approval)

This is why it has me stumped.

I figured that the approval workflow would compensate for the content
approval required by the versioning  setting.
Every time I or the designated user  approves an entry the "created"
timestamp is updated to the current time "modified by" to "system account"
and still comes up for approval. I have tried disabling/enabling other
workflows I am running on the item  as well but to no avail.
