Alert message - Fiel Deleted by (unknown)

I am Share Point Portal Administrator and facing a problem plz let me
solution if anybody know.

When a site user upload a document in document library

The alert message show ..... 456789.xls
<http://iflportal/sites/ppt/ndm/NDM%20Reports/2006/NDM%20December%202006
/456789.xls> was added by Maqsood at 12/15/2006 4:14 PM.

But

When any document deleted from any document library

The alert message show .... 456789.xls was deleted by (unknown) at
12/15/2006 4:16 PM.

Why (unknown) shows when delete any document, how it show user name when
delete document.

Please let me if anybody have solution of this problem.

You didn't say which version of SharePoint you're running, so I'm going
to assume WSS v2.

Do you have it completely patched? This problem was first in WSS v2
SP1, I believe. It's mentioned here,
support.microsoft.com/default.aspx/kb/890930.

The alert get sent as the result of an Event in the document library, Insert,
update, check out, check in, delete,... The problem is that there is no longer
an item in the library for the alert to get the information from. I think in the
new version of SharePoint your will be able to create event handlers that can
capture information before an item is deleted.

Yes Share Point Portal Server 2003 with WSS v2.

No any SP is installed ... Let me should I need to install SP1 or SP2 to
resolve this issue.

I would go to Sp2 as it fixes loads of issues

Apply to all servers web front end first

Actually, there is a fix for this issue of "Deleted by unknown",
that is supplied by Microsoft. If you open the ticket with them,
they should provide you with the fix. My company was given a
personal fix, which I believe should now be public as we had this
deployed to our environments a little less than 2 years ago.
