Adding a Hyperlink or Picture column in MOSS 2007

Thought I might try this question once again with a better explanation.

I'm adding a Hyperlink or Picture Column to a list. The hyperlink is
in another application and will always be the same with a variable
appended to it. I would like the web address to be pre-populated with

notes2.con.southco.net/.../?SearchView&query=

The Description would be filled in by the user and be appended to the
above link. When the New Item form is filled in the result in the List
view would look like

notes2.con.southco.net/.../?SearchView&query=P2007-0164

Does this make sense and is it doable?
