404 NOT FOUND

I'm trying to create a custom master page for my WSS site. For
this, first I created a Site Definition and the from that I'm trying
to create a new custom master page. BUT I'm getting an error as "404
NOT FOUND. This is how I'm trying to create a master
page:
1. Copied the default.master page from ~12\TEMPLATE\GLOBAL to my
site definition folder (ie., ~12
\TEMPLATE\SiteTemplates\MySiteDefinition) and renamed it
as "custom.master"

2. Then, I opened the ONET.XML file from my site definition folder
and added the following code:

<Project AlternateCSS="/_layouts/Custom/AMLI/core.css"></Project>
<Project CustomJSURL="/_layouts/Custom/AMLI/core.js"></Project>

<Configuration ID="1" Name="Blank"
CustomMasterUrl="_catalogs/masterpage/custom.master" />

And after the </Configurations> node,
<Module Name="CustomMasterPage" List="116"
Url="_catalogs/masterpage" RootWebOnly="FALSE">
<File Url="custom.master" Type="GhostableInLibrary"
IgnoreIfAlreadyExists="TRUE" />
</Module>

3. And in custom.master file, I added the following under <BODY> :
<h3> MY MASTER PAGE </h3>

4. And in default.aspx page, I added the following in the very first
line.
<%@Page language="C#" MasterPageFile="~masterurl/custom.master" blah
blah blah %>

5. And finally I did the iisreset.

After all these steps I created a new site using my site definition.
But I'm getting an error as "404 NOT FOUND".
